Japan Health Insurance Foreigners 2026: Skip It and Lose Your Visa

Starting June 2027, Japan health insurance foreigners who have not been paying their National Health Insurance premiums will face visa renewal denial. This is no longer a rumour — Health Minister Ueno Kenichiro formally announced the policy on November 4, 2025, and it is now being implemented. If you live in Japan and have been skipping or delaying your Japan health insurance foreigners payments, you need to fix this immediately. Your visa — and your right to stay in Japan — depends on it.

Japan health insurance foreigners: the 2027 visa denial rule explained

From June 2027, immigration authorities will in principle deny visa renewals and status changes for foreign residents who have failed to pay their Japan health insurance foreigners premiums despite repeated notices. The Ministry of Health, Labour and Welfare has confirmed this applies to National Health Insurance (NHI / 国民健康保険, kokumin kenko hoken) as well as national pension contributions. Japan’s immigration authorities will have direct data access to your payment status when you apply to renew. This is not a minor administrative warning — unpaid Japan health insurance foreigners premiums will be treated as a reason to refuse your stay in Japan.

Japan health insurance foreigners enrollment is mandatory for all foreign nationals staying in Japan for more than three months. This applies regardless of your visa type — work, student, dependent, or long-term resident. Japan has two main types of health insurance:

  • Shakai Hoken (社会保険) — Employee Health Insurance: Your employer enrolls you automatically if you work 30+ hours per week at a company. If you are enrolled in Shakai Hoken, you are compliant with Japan health insurance foreigners requirements and do not need to enrol in NHI separately.
  • Kokumin Kenko Hoken / NHI (国民健康保険): Required for self-employed, freelancers, part-timers under 30 hours, students, and anyone not covered by employer Shakai Hoken. You must register at your ward office within 14 days of arriving or changing status. This is the Japan health insurance foreigners system most commonly missed by expats.

Japan health insurance foreigners NHI premiums are calculated by your ward office based on your previous year’s taxable income in Japan, your age, and your municipality. For someone earning ¥4 million per year in Tokyo, NHI premiums run approximately ¥280,000–¥380,000 per year — roughly ¥25,000–¥32,000 per month. For someone in their first year in Japan with no prior Japanese income, premiums are at the minimum rate — approximately ¥2,000–¥5,000 per month depending on your ward. In return for Japan health insurance foreigners NHI coverage, you pay only 30% of all medical costs at any hospital or clinic in Japan — a significant benefit given Japan’s high-quality healthcare system.

Japan health insurance foreigners: what happens if you don’t pay?

  • Before June 2027: Unpaid Japan health insurance foreigners premiums accumulate as debt. You receive escalating notices from your ward office. After multiple missed payments, you may be issued a limited coverage card (tanki-yo hoken shomeisho) meaning you pay full costs upfront and claim reimbursement — removing the practical benefit of coverage.
  • From June 2027: Unpaid Japan health insurance foreigners premiums will result in visa renewal denial. Immigration will check your payment history directly. Being behind — even by a few months — could end your legal right to stay in Japan.
  • Property seizure: Municipalities that have adopted prepayment rules can seize property from residents with chronic non-payment of Japan health insurance foreigners premiums.
  • Pension non-payment: The same June 2027 rule applies to national pension (国民年金) non-payment — both NHI and pension compliance are checked at renewal.

Japan health insurance foreigners: August 2026 copay changes

Japan health insurance foreigners copay caps are also changing in August 2026. The monthly out-of-pocket maximum (高額療養費制度, kougaku ryouyou hi seido) that limits how much you pay for expensive hospital stays will increase by approximately 4–7% across all income tiers. For example, the middle-income cap rises from ¥80,100 to ¥85,800 per month. Japan will also introduce its first-ever annual out-of-pocket cap of approximately ¥530,000 for middle-income Japan health insurance foreigners enrollees — a new benefit for anyone facing multiple expensive medical months in a row. A second reform in August 2027 will split the current 5 income tiers into 13 finer brackets for more precise Japan health insurance foreigners premium calculation.

Japan health insurance foreigners: My Number Card integration from June 2026

From June 14, 2026, Japan is issuing a new integrated Special Residence Card that combines your Residence Card and My Number Card into a single document — relevant to Japan health insurance foreigners who currently use their insurance card separately. Medical facilities across Japan are switching to My Number Card-based check-in for Japan health insurance foreigners. If you do not have a My Number Card, you can request a free Eligibility Confirmation Certificate (資格確認書) from your ward office. Important: unlike Japanese nationals whose My Number Cards are valid for 10 years, your card expires when your visa expires — after every renewal, visit your ward office to update the card’s expiry date.   • Check your enrollment status: Visit your ward office and confirm you are registered in either Shakai Hoken or NHI. Japan health insurance foreigners who changed jobs, went freelance, or moved wards may have fallen through bureaucratic gaps.
  • Pay any outstanding premiums immediately: Contact your ward office health insurance division, explain your situation, and arrange a payment plan if needed. Ward staff can check your full Japan health insurance foreigners payment history and help you catch up.
  • Set up automatic bank transfer: Japan health insurance foreigners NHI can be paid via automatic bank debit (口座振替) from your Japanese bank account — the simplest way to never miss a payment again.
