Since April 2026, a child and child-rearing support levy of 0.23% rides on Japan's public health insurance premiums. Here is what it costs per month, who pays it (children or not), when it shows on your payslip, and what it funds.
Japan child allowance 2026: ¥15,000/month for children 0–2, ¥10,000/month for 3–15, extended to age 18 from October 2024. Foreign residents fully eligible regardless of nationality. Tokyo 018 Support adds ¥5,000/month per child under 18. A family with 2 kids could save ¥1–2 million yen per year — most expats don't know they qualify.
